Most people complete the process over approximately 4–6 weeks, with flexibility to allow time for reflection between conversations.

What's included:

  • Your Personalised ACP Summary (see example below). A clear guide that captures your values, priorities, and wishes — written in your voice, ready to share. This becomes the reference point for anyone making health and personal care decisions on your behalf.

  • Guidance your decision-makers can actually use. Alongside your full Summary, you get a one-page Emergency Quick Reference for first responders, and a decision guide written for the person who may one day have to speak for you. Most planning stops at recording your wishes. This prepares the people who'll have to act on them.

  • 30-Day Revision Period. Once your ACP Summary is drafted, you'll have 30 days to review it and request revisions to make sure it accurately reflects what we've discussed.

  • Optional SDM Session. Book a facilitated conversation with your Substitute Decision-Maker(s) to walk through your ACP summary together. They get clarity on their role. You get peace of mind. Available as an add-on.

  • Annual Reminder. Each year you'll receive a reminder to consider whether your ACP summary still reflects your wishes. A formal annual review and update is available as a paid service if you'd like one.

  • Compassionate, Expert Guidance. Every step of the way, you’ll work with someone who understands different sides of the healthcare system — as a patient, family member, and volunteer.
